Vacuum drainage collection unit

Hi,
I have a question about HCPCS A7048: Vacuum drainage collection unit (mainly for those with pleural effusions). I am doing market research and it seems that two different companies are getting wildly different reimbursement for the products that fall under this code description. The Medicare ceiling is about $45, with private insurers paying lower. However, some companies are getting between $70-95 for these kits. Can anyone shed light on what the company getting higher reimbursement might be doing? Are there additional codes to add to this one that might make the difference?
