Billing for Forms

One of our providers saw a patient just for filling out forms for patient's life insurance; he also did a blood draw, but said there are no health issues at this time. The only dx he gave is V70.0 for the CPT® 36415 and noted that he wants to bill for the forms; can that be billed? I know L&I has specific forms we bill for but I'm not aware of any code for insurance forms. Also we are a Rural Health clinic and I'm not sure how that plays into it or whether it affects it at all. Help please!

CPT® 99080: Special reports such as insurance forms, more than the information conveyed in the usual medical communications or standard reporting form
Notes: (Do not report 99080 in conjunction with 99455, 99456 for the completion of Workmen's Compensation forms). I am not sure the carrier will pay this (likely not) in the past, I have worked with practices that charge an administrative fee for services of this nature. We did not file to the carrier, it was outlined in our financial policy which the patient signed and the fee ranges from $25-50 and was collected prior to the completion of the forms. It worked for well for us at the time.
