What codes should I use!?!

My doctor did a laparoscopy removal of the left ovary & fallopian tubes as well as lysis of adhesions.

I believe I need to use 58661 w/ 59 mod and 58660. Not feelling 100% sure on this though! Can some please confirm for me if this is correct.

According to the CPT® book 58660 is lap. surgical with lysis of adhesions and
58661 is underneath which I think is a apart of 58660 because it says with removal of adnexal structures.........

58661 includes moderate lysis. The 58660 is not billable in addition to the 58661 if the payer follows NCCI bundling, and you cannot over-ride the bundle with a modifier.
