Physical/B12 injection

How would a claim be coded if a patient received a B12 injection at an appointment for an annual physical? The B12 was given for "malaise and fatigue". I had this happen one other time and the payer bundled the payment of the physical with the injection and I appealed. It got paid. I just want to avoid having to appeal again. Does a modifier need to be used? I just cannot remember. Thank you so much to anyone who can assist!

J3420 + 96372 with Dx 780.79; add modifier 25 to the "physical" E&M with the appropriate V7x.x code for the physical.
