CRNA billing
We are a fairly new anesthesia practice. When we have CRNA does service supervised by the Anesthesiologist what is proper way of billing. Do we bill both the Anesthesiologist and CRNA separate or one bill with QZ modifier to indicate this was a supervision. We had payments from Medicare that's substancially lower from our carriers when we bill it this way.
Any help on this matter is greatly appreciated.

Bill each separate with the appropriate modifiers. CRNA as being supervised and Anesthesiologist as supervising. Should get two checks to reflect the work done by each provider.

Desperately seeking clarification.
I was under the impression that you would only submit 2 claims if the CRNA and/or the Anesthesiologist were employed by the facility and 1 claim if they are in their own provider group.
If what I said is true, how is that reported? Can QK, QY or QX be used on the same claim?
Example:
Charge line 1: 00170-QX
Charge line 2: 00170-QY
