denial of 11404

According to the LCD's for the state of Nevada I cannot use code 11404 with a 13101 for ICD 9 code 706.2.

Did you look at the list of acceptable diagnosis codes for these cpts and this LCD? I would review the patient notes & see if there is an applicable icd 9 code that could be added. I would put the applicable/necessary icd 9 code under pointer 1 and then icd 9 706.2 under pointer 2 & do a corrected claim/rebill.
