Cardio Coding Help Please

Listed below are the procedures on the OR report 1. Left heart catheterization 2. Left ventricular angiogram 3. Selective catheterization and angiogram of the aortic arch 4. Selective catheterization and angiogram of bilateral carotid arteries 5. Selective catheterization and selective angiogram of the intracerbral vessels

I believe Lines 1-4, should be coded with 93458 (26,59) 36215 (59) 36216 (59) However I am not sure about line 5. I was thinking 36223 (59)

Any help on this would be greatly appreciated. Thank you -
