CRNA billing insurances

I am new to the business and I'm currently working with 2 CRNA both are located in Maryland and work independently. Neither person knows what rate to charge for insurance companies. Both agree that the conversation rate is used for medicare / medicaid, but both are wondering if there have the right to have there own rate. Meaning when you take the base value unit and the time unit can it be X by there rate? ex: $45.00. If so does anyone know the average rate the CRNA use in Maryland.

Would anyone know whether or not the difference in the amount charge and the amount received can be written off the CRNA taxes?

I am sure there is a law against price fixing, so I'm not surprised no one has responded to your query.
