replacement of ankle hardware

The doctor removed the ankle syndesmotic fixation screw and
replaced with Anthrex TightRope fixation through the same incision(slightly
enlarged).
How would this be coded?
This appears to be done sometimes with a syndesmotic fixation screw
in anticipation of the screw's possible breakage.
