E&M and Testosterone injection for est patient

Established Medicare patient wants to get his testosterone shot on the same day as his regular follow-up. Will Medicare pay if we use mod 25 on the office visit with a different dx code than the one we use for the shot J1070 and the injection fee 96372? Can we use only one dx 257.2 and still get paid for both the med and injection?

If E/M medically necessary and documented, no reason why you would not get both. Modifier -25 can go on E/M code
