Physical

Patient comes in for Physical only but Dr has obesity, diabetic and obesity counseling. Dr. does not have an E/M noted on superbill. How would this be billed?

With the preventive visit code, the counseling on risk factor prevention would be part of preventive code. If no management of chronic condition then why an E/M code? It is really difficult to say without the actual documentation. There must be a significant problem to carve out an E/M code as well as a preventive visit code.
