coding remicade infusion?

What codes would be appropriate?
IV was placed at 845a, Remicade infused from 1010a to 1215p, NS at 850a and IV D/C at 1315p?

Code only for time spent actually infusing the patient. Use 96413 for the first hour and 96415 for each additional hour (e.g. 2 hr. is 96413, 96415 x 1). Hydration is bundled into the Remicade infusion unless Hypovolemia or dehydration is present, documented, and an order written.

Alternatively, some may prefer use of CPT codes 96365 (IV infusion, for therapy, prophylaxis, or diagnosis (specify substance or drug); initial, up to 1 hour) and 96366 (IV infusion, for therapy, prophylaxis, or diagnosis (specify substance or drug); each additional hour). 96413/96415 is used for ALL Payers on Remicade. I am at the largest private infusion center in the country and have been for over six years.
