endocrinolog not receive full paymant for visits 99215 level5 should I copay?

I have Medicare and Blue Cross Blue Shield.I am patient since three years and never copay

You should first check with Blue Cross Blue Shield to see if your policy has changed. Just because you have 2 insurances doesn't mean you won't owe anything. Medicare has a deductible and your secondary could also have a deductible which could be a reason you would owe something. Also, there are hundreds of Blue Cross Blue Shield policies and without knowing what yours is, nobody would be able to definitively answer this question.

I have Care First Blue Choice BC/BS Plan 080/580
I do appreciate your advice how to avoid that extra $62 payment for simply, regular visit.
I do not remember (may be because I am 83 Y/old) any visit longer than 30 minutes.
For now I decided to stop see this doctor anymore while I can not afford his expensive service.

Just remember, if your doctor is a Medicare participating provider, they cannot charge you more than Medicare allows. So if your deductible has been met, you would only owe 20% of what Medicare allows. Since I don't know your BC/BS benefits, I cannot say what they would pay.
