office visit and minor surgical procedure

Can the doctor bill only for an office visit (E/M) to include the minor surgical procedure with the E/M? For example patient is seen in dermatology for multiple skin issues, has full exam and doctor informs patient if they would like to have the AK procedure done at time of visit rather then having the patient come back for the procedure, patient consents to have several AK's done, can the AK's that were done be included in the office visit, where EM visit would only be billed but not the AK's billed?
