Is it a requirement for the anesthesia record to contain a box for anesthesiologist to mark stating present at induction and emergence?

Yes. The form used should indicate start and end time for anesthesia, the OR time in/out and Surgical start/stop time. the CRNA's time in/out should be the latest on the record indicating the anesthesia specialist stayed with the pt until handed off in recovery.
