E/M Question
We are the Medical Director of a Free Standing Psychiatric Sanctuary. There is
also a Psychiatric Group with a lead Medical Director. My questions are as
follows: 1: When the new patient is admitted to the Sanctuary the Psychiatric group want my medical group to do an initial medical H&P and then see the patient on daily rounds for medical reasons only. Can we bill and if so which CPT® E/M codes would we use. 2: Can a NP do and initial H&P in this type of facility?
Have posed this question in many places and have never received an answer. I do not know where else to turn. Thank you in advance.

I would imagine you would use the same codes for Inpatient visits. 99223, 99231-99233. Our PA's do the intial H&P in the hospital for us.. but our physicians must also see the patient that day and sign off on the records.
Don't quote me on this. I have never billed for psychiatry claims.

Yes, you would use the usual hospital inpatient E/M codes. Yes your NP can perform the H&P and make rounds. If your NP has their own NPI, they can bill on their own. If the MD examines pt the same day he/she must document same and sign off on the note agreeing with the NP's assessment/plan. The charge will then be under the MD's NPI.
