Are the MUEs specifically for Medicare or for all payers?

MUEs are developed for Medicare nationally; however, we have recently noted dosage limits on at least one chemo drug (UHC) which we have had to appeal. And, Colorado Medicaid also has secret unit limits that they do not disclose on their fee schedule or web site. We have to call and specifically ask per drug.
