initial ob visit

When a patient is seen for her initial prenatal visit with the doctor and I bill for a pap smear and GC/Chlamydia cultures, do I use v72.31, v74.5 as well as v22.0? AND, can I bill 99395 as well as 99212 for the cultures or is this inappropriate??

We do this alot with our new ob's depending upon ins. as long as the doctor did not start an antepartum visit. I bill the 9939X and 81002 (if done) with the V72.31 then 81025 (if done) with V72.42. The std is part of the pap, I would only bill the office visit if she came in with pelvic pain, irrg.menses etc. with a mod.25. Anthem I have to put a 25 on both 9939x and 9921x or they will bundle them both.. hope this helps
