colonoscopy
How do I code 45380, 96365, 96375, J7050? Insurance is denying stating that the infusion and drug codes are included with the colonoscopy. Do I use a modifier?

Where are you doing this procedure? In a facility, the facility bills for the medications and injections/infusions.
