58100 & 99386

Climed 58100 & 99386 together and 99386 is not getting paid. Why?

Did you use a modifier 25 on the 99386? I bill these codes together all the time and do not have a problem usually. Hope this helps!

99386 and endometrial biopsy

There is no coding rule that says these should not be paid together. The preventive medicine service will need modifier 25, and of course, different diagnosis codes.

But, some payers will deny these. You may have to appeal. And if it's Medicaid, abandon hope.

Hi ,

Most of the period we get paid both the service from insurance when billed with Mod-25 for physical.One time i billed Modifier-57 for Annual depends the record, physician decide to proceed today this surgery.
