coronary calcium and cardiac risk assessment "consultations"

I have a group of Cardiologists who are doing the following and we're looking for guidance on how/if we can bill for this service:
1. Patients are either self-referral or recommended by PCP to have the coronary calcium CT scan done, usually to assess underlying risk for heart disease and rarely for personal history issues.
2. Patient goes to IDTF for the actual scan where the "official" interp is done by a Radiologist.
3. Patient immediately leaves IDTF with a copy of this "official" interp and brings to Cardiologist who reviews and explains the results, does a comprehensive cardiovascular and lifestyle evaluation and if necessary recommends additional testing (rare).
Questions:
1. Is this a billable service by the Cardiologist and if so, using what CPT® code? (dx is V81.2)
2. Cardiologist wants to call this "non-covered service" and bill patient out of pocket for their visit - is this appropriate?
