90945 vs. 99233

Since CPT® code 99233 is not separately payable with CPT® 90945, would it be appropriate to bill 99233 instead of 90945 if the documentation supports the E/M level? I am questioning this as the reimbursement for 99233 is higher than 90945, and the physician feels that his documentation supports the reimbursement for his e/m service.
