Number of Diagnoses or Treatment Option

If the physicians orders a Nebulizer treatment or chest x-ray on a new patient does that allow me to check off the box indicating "New Problem (to examiner): add work-up plan. Or is that box only to be used when we are requesting work-up outside of the office not on the same day as patient's encounter
