99070 &97602 billing

Hello,
1. Are these codes covered if performed in a nursing facility?
2. If so, who must bill - the provider or the facility?
3. Billed via UB or HCFA 1500 form?
4. Fee schedule?
5. Can only find LCD for Wisconsin? Is it even covered in OHIO?
