dental services in doctors office

I'm having a bit of trouble figuring out how to code services done by a NP, MD or DO in office that are dental in nature.

For instance, application of Coe-Pak periodontal paste to stabilize an avulsed tooth
or for a gingival laceration (they usually use it instead of suturing)

For a nerve block using bupivicaine and epi for treatment of tooth pain (64400?)

Application of zinc oxide/eugenol temporary cement (Temrex)

or application of calcium hydroxide (Dycal)

Dental and emergency care are nowhere near us so our docs often do temp services until the patient is able to make it somewhere else.
