NursingHome Billing Question ?

Dear Friends,

One of our providers is a medical director for a nursing home and hence he does nursing home rounds at the facility every month. This month we want one of our Nurse Practitioners do the rounds at that facility (She has her own Medicare Number, we bill her office visits under her own number) how should we bill for her visit, whom should be we as Supervising Provider when we send the claim out. (the provider will not be in the nursing facility when she is doing the rounds), is it possible to select her name as Rendering Provider and our MD's name as supervising provider and still get paid at her rate (I think it is 85%), can anyone with this knowledge please guide me so that we code it the right way.

NP/PA can see Medicare patients in a SNF (POS 31) without supervision. The MD must do the initial eval and NP/PA can see patient thereafter. Some providers do not credential NP/PA therefore they must be supervised. Need to look at your state statutes and individual insurance company policies for guidance.
NP/PA can do initial eval for Medicare patients in a NH (POS 32) and do follow-ups.
Might want to start with Medicare rules on incident to. Some of the above is addressed there. Also MLN Matters number SE1308 might help.
