CPT® - 84999 with 2 Units

Hi,

We have more than 300 claims being denied across our hospitals on this procedure code.

Across the board we have a denial for this CPT® stating " This is an Unlisted Procedure and has been flagged and will not be processed. Only these claim can be appealed". Even after appealing, the claims are getting denied with the same reason.

We are even billing the claim with the description in Block-19 "1 unit = Fructose Sensitivity Assay, 1 unit = Glucose/Insulin Metabolism".

We are getting freaked out on how to fix this issue. Please let me know if the way i bill these claims are correct or is there a solution to get rid of this denial.
