Infusions and Injections -- Facility Coding

I am having a debate regarding two issues wth infusion and injections, and I hoping you all can point me in the right direction. I have two questions:

1. When a patient has two IV accesses for infusions and injections, are coders limited as what can count as initial codes for each IV access. For example, IV 1 has an IV infusion of Levaquin and 2 IV push of Zofran, but the IV infiltrates. IV 2 was placed in the contralateral line, and there were 4 IV pushes in this line (2 Zofran and 2 Dilaudid). Is this the correct coding?

2. The patient received Solu-Medrol IV push at 1305. At 1445, the patient received an IV infusion of Solu-Medrol that stopped at 1530.
Are these codes correct? 96365 and 96376
