Medical services over the phone

A few of our patients are bed ridden and can not come to the clinic. Therefore, the Dr. provides his services over the phone by either talking to family members or the nurses at nursing homes. He refills medications and order new services after verifying patient status, response to treatment, etc. None of these services have ever billed. Can I bill for them? What code do I use? What are the guidelines? Thanks for your help, Elia
