Having a difficult time locating Medicare rules regarding group psychotherapy. Getting denials "payment adjusted because the payer deems the information submitted does not support this many/frequency of service" and "service denied because payment already made for same/similar procedure within sert time frame"

What are the rules. Psychicatrist sees group once per week.
