It is your Contractual Obligation adjustment down to the agreed reimbursement.

It means it is the facility's contractual obiligation and patient can not be billed for that amount. It should be adjusted off the patient's bill.

What if you are a non-contracted facility for pain management and you receive this type of denial from BCBS? What fee schedule are they referring to?

What codes are you billing exactly? We see those if we bill a procedure with an office visit without the modifier or if we bill and office visit prior to a procedure and the office visit gets paid before the procedure is processed. I think I need a little more info on what you are billing in order to help more.
