Xray report

Our office has it's own xray equipment, so we take our own xrays and the physicians interpret them. In order to bill both the technical component and professional component, do we need to have the xrays report as a completely separate document or can the findings and what xrays were taken be contained within the body of the physicians' reports? I've had one source tell me yes, we must have a separate report and I've had one source tell me no. Please advise.

X-rays Separate Report

Yes, if you are reporting a service that has a professional interpretation to the code, and you are reporting either the global radiology code or the x-ray code with a modifier 26, a separate report is required. A report is a stand alone document and X-rays are considered variable services. The rules are well documented throughout the CPT® book.
