Submitted codes 99234 modifier 25, 93460 modifier 26 , 93571 modifier 26 place of service is 22. please let me know what is wrong.

If you look under code 93571 on page 515 in CPT®, the codes you have listed are not listed there.

Code first ([92920], [92924], [92928], [92933], [92937], [92941], [92943], [92975], 93454-93461, 93563-93564) Double-check your mods are appropriate. For MCR it is packaged into the APC rate(for the hospital anyway).

Ah, I take it back the code is there!
I would find out the reason behind the denial and check your modifiers.

I have been trying to find out how many units 93571 MCR will allow. I am billing 2 separate lines with modifiers 26/LD & 26/RC for each line. But Medicare denied the 2nd line with the 26/RC modifier not supporting this many frequency services.
