Am I correct, if a patient is booked for a procedure only, no visit should be charged? But if a patient is in to see the doctor with a complaint and at the time of the visit it is decided a procedure needs to be done, the correct charge would be ofc visit with -25 modifier added and the procedure?

Yes if you address a complaint different from why you are doing the procedure.
