Reimbursement for CPT® code 76376

Having difficulty obtaining reimbursement for CPT® code 76376. No particular carrier. Most of the time it is denied as bundled. Is there any time that this procedure will be paid?

76376 —3D rendering with interpretation and reporting of CT, magnetic resonance imaging (MRI), ultrasound, or other tomographic modality; not requiring image postprocessing on an independent workstation. With the deletion of code 76375, reimbursement for 2D reconstructions will be bundled with the base procedure code.
Practices that routinely provide 3D rendering may prompt an investigation by the Office of Inspector General. (Do not report 76376 in conjunction with 70496, 70498, 70544-70549, 71275, 71555, 72159, 72191, 72198, 73206, 73225, 73706, 73725, 74175, 74185, 75635, 76377, 78000-78999, 0066T, 0067T, 0144T-0151T).
The ACR indicates the 3D codes should not be used when 3D is not medically necessary.
