Evaluations

I have a disabled population that we deal with and I am always stumped on a code for evaluation of labwork that was done and also evaluation for a new wheelchair as pt maybe going from a manual to motorized ..doc sees pt for 15 minutes and I figure these would come under time but not sure...they dictatate a letter with their recommendations for the pt ..any ideas..TY
