Timeframe for Colonoscopy Procedures

Hi,
If a patient was last seen for a colonoscopy over 10 years ago, had a polyp removed(but not sure what type of polyp it was) Do we bill now for a screening colonoscopy(since it's been more than 10 years) or this a surveillance? This is not a Medicare patient
