The doctor has removed the screw for syndesmotic disruption because
after being in for a period of time they have a tendency to break and
he has done ankle syndesmotic fixation with an Anthrex Tightrope Fixation

V54.0 and if in global period and planned prospectively, modifier -58.

Cannot say because it depends on op note. Removal usually included in insertion. Modifier -58 is staged procedure, if the second procedure was planned prospectively in global period.
