modifier need

We did a 2 week visit on a newborn in the office along with a circumcision and frenulum clip
We know we need a modifier, or modifiers, but unsure what to attach and what to attach it/them to

Put modifier -25 on the E/M code when you also do a minor procedure in office (0 or 10 day global)
