Multiple procedures during colonoscopy

I have read posts here from years past regarding the ability to charge multiple procedure codes for a single colonoscopy. My facility has informed me that this is no longer the case. For instance, if a hot biopsy and a snare (45384 & 45385) were performed during the same colonoscopy procedure on different polyps, can they both be charged with a 59 modifier? What about a biopsy (45380) and a polypectomy?

The code assignment is determined by the physician's documentation of the procedure performed or the method used to remove the tissue, lesion or polyp. Example:
The physician performs a colonoscopy, a submucosal saline injection and then uses bipolar cautery to remove a large colorectal polyp and then uses a bipolar snare to remove a second polyp from the ascending colon. 45381, 45384; 45385. Because two different polyps were removed using two different techniques, two separate codes should be assigned. In this instance, code 45384 is reported for the bipolar cautery of the polyp and 45385 is reported for the bipolar snare removal. Modifier 59 should be appended to codes 45384 and 45385.

Yes 45380/59 for cold biopsy removal.
