Modifier

Our office did a procedure in the operating room as an outpatient and used procedure code 12001 and have had the claim rejected secondary to not having correct modifier. Can you tell me what the appropriate modifier would be? I tired 24, 25, 51, and 57. Thank you.

Was this done during a global period: if so, what CPT code and what date was it performed.

If another service on the same day. what CPT code.

Without that information, it isn't possible to know what modifier is appropriate.
