How do we get the E&M code paid when billed with a 94760 code?

We have recently started seeing payments on a 94760 code when billed with an office visit code. However, seems we are not getting paid for the office visit code. Has anyone else experienced this and how do you get both paid? Do you have to use 2 separate diagnosis codes?

Any feedback is much appreciated.

Are you using a modifier? I have to use a 25 mod on my E&M when I do our testings. Just a thought.
