modifer T4?

CPT® 29297Corrj Hallux Valgus, 27691 TR/Trnspl 1 tdn w / musc re dirion/rerouting dp 28008 fasciotomy foot /toe= to correct congenital left 5th toe- my question
Do I put T4 modifier on all CPT®? 29297 T4, 27691 T4, 28008 T4 ?
Or, 29297, 27691 28008.T4? Please explain your answer... source?

T 4 modifier answer

Per CCI edit - 29297 & 27691 do not allow modifier T4... 28008 allows modifer T4.

Level II (HCPCS/National) "T modifiers" are used to identify surgery performed on specific toes and are found at the back of Appendix A in the Current Procedural Terminology (CPT®) book. They only apply to the phalanges and are not used to identify metatarsal work
