Punch Biopsy Coding
Punch Biopsies were done on 3 separate sites during the same visit. Would I list the punch biopsy code 11100 3 times using a 59 modifier on each CPT® code and also would need to add Left and Right modifiers to 2 of the diagnosis codes ?

11100 is for a "single" lesion. Any addt'l lesions use 11101. If, you're doing multiple biopsies on a single lesion I do not believe you can bill more than once. I do not recommend using LT,RT unless it is actually being performed on the other body part.
