Coding from the Operative Note

Good Morning,

I have been told coding strictly from the operative note is fradulent. Could anyone provide information regarding this?

I haven't heard of that. I only code from the operative note because my doctors don't code anything. In addition, most of the time that they tell me they did one thing, I find that there were other things done when I read the operative report. Example: doctor says they repaired an incisional hernia. I have to read the operative report to see if they used mesh. Mesh is billed as a add-on if they used it.

If you were using the operative note to code for something other then the operation then I would agree that you cannot code from the operative report.

I totally agree with the response. Docs frequently don't know what they can or
cannot bill for. Always use the op note. It's the safest & most correct way to
bill procedures or surgeries. Good work becka95
