J1071 not being paid

I billed the J1071 and the insurance company denied it stating that "Missing/ invalid/ incomplete patient's diagnosis or condition". They paid for the injection but not the testosterone. I used dx 257.2 for both of them.

Any suggestions on how to get this paid?
