AS Modifier for a PA assisting during surgery.

Not being a coder I am looking for what are the documentation requirements for a PA assisting during surgery in regards to the operating note?

Some offices have indicated that the name of the PA who is assisting at the top of the operative note is sufficient.no mention of their participation in the operative report . Some feel the PA's are technically required to be under the supervision of the MD so the MD's documentation without the mention of the PA is what they have always used and submitted in medical reviews with no problem?

[When another MD assists and we use modifier 80- "documentation should clearly state what the assistant surgeon MD,did during the operative session and be able to take over the surgery should the primary surgeon become incapacitated"
as the AMA Coding with Modifiers book indicates on page 223].
