Re procedure code 27511

Can you please help me with this billing question ?
Iam billing procedure code 27511 to caresource with dx code 996.44 is this the correct dx code to bill with that procedure I will alson be using the modifier 78 as the dr did a surgery on the patient on 5/22/15 procedure code 27447.
