proper sequence of codes

Is there any rules that address the right way to sequence codes? If they are out of sequence the claim is denied. I know the primary code (reason for encounter) is first, but after that I'm unsure about what order should be followed Out patient encounters are the ones that are confusing. Thank You

You should always sequence per coding conventions and guidelines as indicated in your ICD-9 code book. These are found in the front of the book, as well as throughout the tabular section. These are your coding standards.
Hospital coders are accountable for proper sequencing. Although payers have been somewhat lenient with physician coding in the past, are increasingly requiring the same standards from physicians.
I always include this type of information in my revenue management education classes.
