Billed 45384 and 45380 , insurance will not paid. Stating it's bundled. Is there a modifier i could use or appeal with medical records?

The CCI edits do bundle 45380 into 45384. It has an indicator of "1", this means code 45380 can only be billed (with -59 modifier), if it is not performed on the lesions described in code 45384. If the biopsy was performed on a separate and distinct lesion, then bill code 45380-59 (single or multiple biopsies).
